Admissions and Referrals

Referral Process

​The main component of a Wayside House referral is completion of the Global Appraisal of Individual Needs (GAIN) and/or the Admission and Discharge Criteria and Assessment Tool (ADAT). These assessments can be completed by qualified counsellors in a variety of addiction service settings. For example, applicants who reside in Hamilton can have a GAIN assessment completed through Alcohol, Drug and Gambling Services. 

The minimum required documents for a completed referral are:


  • GAIN or ADAT, including consents to release information by referral source
  • Standardized Residential Services Referral Form
  • Medical Evaluation Form

The assessment package should then be emailed, faxed or hard copy mailed to Wayside House for our review. Please note that we require all assessment and referral packages be complete in accordance to the Standardized Referral Package for Residential Services as endorsed by Addictions and Mental Health Ontario. 

In addition to the above documents, Wayside House may require additional documents to ensure a safe and appropriate referral. These can include: a current prescription list direct from your pharmacy; legal documentation detailing prohibitions, summons and/or conditions; and relevant psychiatric documentation.

Admissions Criteria

Wayside House makes every effort to accommodate incoming referrals; however, there are certain admission criteria that are crucial to our program vision. Potential clients must be:

  • Men, aged 18 and over, experiencing substance dependency
  • In a physical and mental condition that allows full participation in a structured, intensive treatment program; this determination is made carefully and with reference to all relevant medical documentation 
  • Committed to abstinence-based recovery goals
  • Willing to contribute to and foster an environment that is respectful, recovery- and solution-oriented, and free of all kinds of discrimination. ​​
