Admissions and Making a Referral

ADMISSIONS and REFERRAL PROCESS

​The admission process at Wayside House requires that the Admission and Discharge Criteria and Assessment Tools and/or the GAIN be completed with the potential client. 

This assessment can be completed by a counsellor in a variety of addiction service settings. The assessment package should then be  emailed, faxed or hard copy mailed to Wayside House for our review. Please note that we require all assessment and referral packages be complete in accordance to the Standardized Referral Package for Residential Services as endorsed by Addictions and Mental Health Ontario. 

Minimum required documents for a completed referral and entry onto wait list are:


  • GAIN or ADAT Assessment Tools including consents to release information by referral source
  • Standardized Residential Services Referral Form (download here)
  • Medical Evaluation Form (download here)
  • Upon receipt of all required documents, Wayside House may require additional documents to support a safe and appropriate referral.  These may include  current prescription list direct from pharmacies, legal documentation detailing prohibitions, summons and/or conditions and/or psychiatric documentation where indicated.

ADMISSIONS CRITERIA

  • Men, ages 18 and over, experiencing substance dependency.
  • Participants' physical and mental health must be stable enough to fully participate in a structured, intensive treatment program; as evidenced by their physician's report form and any other health reports that may be required.
  • Recovery goals must be compatible with ‘abstinence-based’ treatment philosophy. Wayside House of Hamilton is striving towards being recognized as a respected leader in ‘abstinence-based’ treatment.
  • Wayside House continues to work with and provide access to Opioid Agonist Therapy (OAT) including Methadone, Suboxone and Sublcade
  • Persons admitted to the program agree to embrace and foster an environment that is respectful, recovery and solution orientated, and free of discrimination. ​


  Effective October 6th, 2021, all admissions MUST be double vaccinated for COVID-19.  Clients must also be receptive to          boosters if recommended by Ontario Public Health.

RESIDENTIAL TREATMENT

​What to Expect from the Residential Treatment Program
  • The main program is composed of men specific educational sessions, discussion groups and peer support groups. In addition, optional outside AA, NA or CA meetings are attended in the local community. Other support groups are also recognized including Men for Sobriety.
  • Individual sessions are involved in our program as well, and are conducted throughout the treatment stay. 
  • The residential living aspect of Wayside House provides elements that are of additional benefit to our clients. ​​Living in a group setting can be demanding as well as challenging, but it can also be a lot of fun and educational. Keeping Wayside House neat, clean, and homelike is a big part of residing in a group setting. Participation in various household chores is important and mandatory. Both private and communal areas are the responsibility of everyone.   Staff and program graduates teach clients how to cook nutritious and cost effective meals. These activities also lend themselves well to learning life skills that are easily transferable and assist in adjusting to life outside of Wayside.
  • Every week a structured social night and movie night are held at the House. These events are client directed and facilitated, however staff also participate and join in on these activities as well.  This aspect of Wayside House emphasizes that there can be enjoyment in the recovery process and demonstrates a healthy balance. Recovery is a lifestyle that can take considerable work, but must also be enjoyed in order for one to achieve the highest level of success. ​
